By attending Music City Multiples (MCM) events, I give MCM permission, in connection with any photographs and/or video taken during MCM events of me, my children, or anyone for whom I have responsibility, related to my/our experiences with MCM. Music City Multiples has the right to use and reuse photographs and/or video inn whole or in part, either by themselves or in conjunction with other photographs and/or video in any print, electronic, or social media and for any purposes whatsoever including without limitation, all promotional or publicity uses. This authorization extends to all languages, media, formats, and markets now known or later discovered. Further, you release MCM, its officers, directors, employees and agents from any and all claims, actions, and demands arising out of in conjunction with the taking and use of said photographs and/or video. I understand I will not be compensated for these images. All group photos will be published. I understand that my/my family's presence in group photos from events may deem my request impossible to fulfill, and further, if you have any hesitation about our inclusion, I agree that my family should abstain from being in group photos. This release does NOT apply to member's personal photos. MCM will not post personal information, children's full names or details that are not specifically related.

I voluntarily seek to participate in the activities run by Music City Multiples and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while participating. I acknowledge that Music City Multiples cannot guarantee that I will not become infected with the Coronavirus/COVID-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from attending in person events. I hereby release and agree to hold Music City Multiples harmless from and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself, my child/children, and/or property that may be caused by any act, or failure to act of Music City Multiples, or that may otherwise arise in any way in connection with my participation in any activities of Music City Multiples, or in connection with the receipt of any services from Music City Multiples. I understand that this release discharges Music City Multiples from any liability or claim that I, my heirs, or any personal representatives may have against Music City Multiples with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection with, my participation in any activities arranged or provided by Music City Multiples. This liability waiver and release extends to the organization together with all board members and volunteers.
